Warranty Transfer

Please Note:

This form is a submission request to transfer your warranty for specified reasons. All submissions are subject to review and approval by Cosmo Management. Submission of this form does not instantly qualify the Warranty as transferred.

A Cosmo Representative will contact you directly to inform you that the submission has been accepted/rejected.

If accepted, the warranty will be transferred as requested for the duration of the original warranty. No further warranty will be provided.

  • The warranty cannot be transferred more than once.
  • The warranty cannot be transferred back to the original purchaser once transferred.
  • The warranty transfer form will not be accepted if submitted by the recipient and must be submitted by the builder / contractor.

Warranty Transfer Form

Please enter the name of the Original Purchaser who is Transferring the Warranty.

Please make sure the Proof of Purchase is a single file, smaller than 1GB. This should include the following:
• Name of the Original purchaser
• Name of the Retailer the Unit was Purchased from
• Date the Purchase was made
• Price Paid for the Unit(s)
